The Stokes County Home Health Agency is a Medicare and Medicaid

Certified agency providing services 24 hours daily, seven days per week

To all residents of Stokes County with in-home needs.

Services Provided Include

· Nursing

· In-Home Aide

· Occupational therapy

· Physical therapy

· Speech therapy

· Nutrition

· IV Infusion

· Social Worker

 

Criteria for Eligibility

· Skilled care need on an intermittent basis under a doctor’s care who

    will give order for treatment

· Homebound or unable to leave home without difficulty
