Communicable Disease/Tuberculosis Control
Family Planning: We provide comprehensive Family Planning Services to any client of childbearing age that is seeking a method of birth control. All Family Planning Services are provided without regard to religion, race, national origin, creed, disability, sex, martial status, age, contraceptive preference or number of pregnancies that resulted in the birth of a viable offspring. Family Planning Services include providing: · Physical history assessments; · Physical examinations & laboratory services; · Medical, nutritional & social assessments; · A prescription for the method of choice; · Treatment, referral & follow-up The Family Planning income eligibility scale will be used to determine fees for Family Planning Services and charges will be made on the basis of the patient’s ability to pay.
Family Planning Education & Counseling: Education is important in maintaining sexual health. Here at Stokes Family Health Center, our nurses are available to provide education and counseling when needed. · Appropriate recommendations to a specialist for infertility · Education on tubal ligations & vasectomy · Temporary methods of contraception for males
Child Heath: Our child health programs goal is to prevent illness through education and immunization. We also provide primary care to newborns up to the age of 21. Routine examinations will be scheduled to help ensure that your child is maintaining optimal health and developing appropriately. Routine child health services include: · Updated history · Growth measurements · Physical history · Hearing & vision screening · Developmental screening · Laboratory screening as indicated (hemoglobin, lead, urinalysis) · Dietary screening/nutritional guidance · Plan of care for problems · Referral/consultation as needed · Age-appropriate counseling & guidance
Adult Health Screening: The Health Center provides primary care for all ages, men and women. Adult health screenings include: · Annual physicals · Sick visits · Lab testing · Exams · Education · Referrals to specialists if needed · Appointments available 5 days a week · Triage nurse available for minor emergencies · Telephone inquiries & a nurse answering service after 5:00 pm & on weekends
Carolina Access Provider: The Stokes Family Health Center provides excellent primary care for Carolina Access Clients. We have quality care provided by several Medical Doctors and a Physician’s assistant with two locations to serve you. We can meet your medical needs whether it is for physicals, sickness or emergencies. If you enrolled in the Carolina Access program and would like to try our services, come by our Danbury or King offices or call 593-2400 (Danbury) or 983-5000 (King) for an appointment to be seen. If you would like to transfer your Carolina Access to the Stokes Family Health Center, we will be glad to assist you with the necessary paperwork.
Health Check/Health Choice: The Stokes Family Health Center offers Health Check/Health Choice, a free or low-cost health insurance for children and teens under 19 years old (21 in some cases). Also, children with one or two working parents earning $42,400 a year or more may qualify. This program also covers children living with grandparents, other family or friends. There are many benefits included with this insurance such as well-child checkups, dental care, hospital care, surgeries, therapies, vision & hearing care and many more. Applications are available at the Stokes Family Health Center. Laboratory: All laboratory test that we perform here at the Stokes Family Health Center are regulated by the Clinical Laboratory Improvement Act of 1988. Daily tests that are performed and the results can be given in the same day are: · Hemoglobin · Urinalysis · Pregnancy testing · Cholesterol screening · Glucose screens · Lipid profile screens · Occult blood · Wet prep · Rapid strep. We also draw tubes for STDS to be sent to the State Lab. Any other tests that are ordered by the clinician are sent to reference labs or to the State Lab of Public Health.
