How Can I Apply for a Program?

You can apply for services/programs by either coming in to the Danbury Office Monday through Friday or the Southwest Service Center on Thursdays. We are open from 8:30-5:00 PM. The Southwest Service Center is located on the corner of Brown Road and Hartgrove Road in King, NC.

You may also apply for Medicaid and/or Food and Nutrition Services by mail. Below are links to the applications. The effective date of your application will be the date the application is received in the Danbury office. Our mailing address is:
